Advanced Care Connect is a care management service brought to you by United Physicians Inc., a physician-owned, physician-led independent provider organization.

Our partners at Medstar Mobile Medic offer a variety of services for our Advanced Care Connect patients including non-emergent but urgent medical need triage, and post discharge hospital visits. Beyond our normal business hours, Medstar will answer your call for any urgent health concerns. They will either immediately send EMS to your home if it is a medical emergency, or connect you with our on call Nurse Practitioner or connect you with a paramedic for a non-emergent, but urgent problem such as minor injuries or mild symptoms that can be taken care of in your home without going to the Emergency Room.

Our partners at Illumia and gentiva know that when you’re dealing with a serious illness, home is the place you feel most comfortable. Their palliative and hospice care reams provide care in your residence including pain and symptom management in collaboration with your primary care provider.

Loving Touch Transportation is offered to all ACC members for non-emergent medical transportation when another forms of transportation is unavailable. They are fully equipped to assist those in need of specialized transportation. They provide kind, courteous and timely service at no cost to ACC members.
